Advance Booking (DVDs and VHS tapes) Request Form

Use this form to book audiovisual materials in advance for your class presentation.

We encourage you to submit this form at least 3 weeks prior to the date you need any given item. This will help ensure its availability. AV items will be checked out in your name for one full week after the presentation date, unless other arrangements are made with Access Services. Just come by the Service Desk to pick up the items. Please return materials on time to avoid any late fees.
